Enrollment Forms

It's time to enroll & re-enroll for the 2022-2023 4-H year! 

Please enroll online (same process as last year) OR stop into office to fill out a paper form in person. 

4-H YOUTH ENROLLMENT & RE-ENROLLMENT:

• Let's enroll online! Click HERE  
• NON-Horse members: Sign and complete this consent form then upload to your online account while enrolling: NON-HORSE CONSENT PACKET
• ALL EQUINE MEMBERS should use this consent form instead: HORSE CONSENT PACKET
• Then go to 4-H Online site: https://v2.4honline.com/#/user/sign-in
• Upload your completed and saved consent form on your 4-H online profile while enrolling, then submit. 
• Then please pay your enrollment fee. $25 per youth until 11/30/22. On December 1, 2022, late fee is added=becomes $30 per youth as of 12/1/22 and after. 
• You can pay with credit card (see below) or mail in or drop off cash or check made out to "CCE Niagara." Mail to Niagara County 4-H; 4487 Lake Avenue, Lockport NY 14094
• If you were unable to upload your signed consent form online during your online enrollment process, please email it to the 4-H office. 

Once your online enrollment has been reviewed, enrollment fees & signed consent forms are received, you will receive an email from “4-H Online” that your enrollment was approved. If you still have questions please email us Niagara4H@cornell.edu

Option #2 Youth Enrollment - Don't want to enroll online? 4-H Youth can stop into the office and fill out a paper enrollment form. Or print one  HERE  - please submit with signed consent form AND payment to 4H office. 

PAPER ENROLLMENT FORM - Includes consent packet within it (unless you are an equine member, then you also have to fill out the equine consent packet, listed above)

• Sign and complete this consent form found HERE  then upload to your online account while enrolling
• Then go to 4-H Online site: https://v2.4honline.com/#/user/sign-in
• Upload your completed and saved consent form on your 4-H online profile while enrolling, then submit.
• If you were unable to upload your signed consent form online during your online enrollment process, please email it to the 4-H office.
• Please watch the 10 minute sexual harassment prevention training video (see below)
• Background checks are completed every 3 years. The office will contact you if you are due. 

Once your online enrollment has been reviewed, signed consent forms are received, and we have confirmation that you watched the sexual harassment prevention video via email, you will receive an email from “4-H Online” that your enrollment was approved. Option #2 Adult Enrollment - Don't want to enroll online? You can stop into the office and fill out a paper enrollment form. Or print one  HERE  - please submit with signed consent form to the 4-H office.

Special Events

4-H’ers and adult volunteers participating in special events (field trips, overnights, camps, etc.) need the following Medical Release, Risk form, and the Code of Conduct form. You do not need the following forms for enrollment—only for special events.
